The Florida Department of Health (DOH) in Pok County announced the launch of its 2025 Community Health Assessment survey on Monday, March 25. According to the DOH, survey results will be used to help understand community health concerns so meaningful and effective improvements can be made. They are encouraging community members to complete and share the survey with friends and family who reside in the Polk County region. "This survey allows us to hear from community members about health and quality of life in Polk County. Data collected from the survey will be used to determine the top health needs for our community. The last Community Health Assessment (CHA) survey, conducted in 2019, has informed health improvement efforts in the areas of behavioral health, access to care, and food insecurity across the county," from the DOH in Polk County.
